Direct PRimary care (DPC) Membership - MOnthly Fee

  • Wellness exams are not able to be completed via telemedicine

* For new members, Simply Well Healthcare requires a 3 month membership commitment. This gives you time to experience Direct Primary Care and build a meaningful relationship with your provider. After 3 months, your membership continues on a month-to-month basis and can be canceled at any time with a 30-day notice.

One Time DPC Membership Fees

Enrollment Fee

Getting started is simple—your first month’s membership fee, along with a one-time $100 enrollment fee, is due at sign-up and must be paid in full before your care begins.


Reinstatement Fee
If you leave the practice and later wish to return, we’re always happy to welcome you back. A $100 reinstatement fee will apply to restart your membership.

Your DPC Membership Includes:

  • Unlimited office visits with no extra charge
  • Extended appointment times (30–60 minutes, not rushed 10-minute visits)
  • Same-day or next-day scheduling
  • Direct access to your provider via phone, text, or secure messaging
  • Chronic disease management (diabetes, hypertension, thyroid issues, etc.)
  • Preventive care
  • Urgent care visits for illnesses or injuries
  • Personalized care plans 


It’s like having a healthcare provider on call—without the traditional red tape.

ACute Visit Non membership - One time fee

Single visits are for one specific concern, such as a cough, cold, back pain, headache, minor injury, infection, or sudden illness.

  • These visits are for one-time concerns only and do not replace ongoing primary care.
  • Chronic conditions, follow-up visits, and long-term care require a DPC membership with Simply Well Healthcare.
  • For emergencies, call 911 or go to the nearest emergency department.
